Have no fear, Bob Hughes at GCD is re-indexing the book. (that's where the additional info comes from, I have said this many times in these forums, but no one ever seems to remember or listen to me). Likely this book was originally indexed from one of those whose who type guides, which obviously can't be trusted to convey accurate information. There is no substitute for laying eyes on an actual copy.
