Just wanted to report that it looks like "This Magazine Is Haunted 012" under Fawcett Comics is actually the Charlton volume 2 version of the same number.

Not it's pretty clearly the Fawcett version as can be seen in the indicia on the first inside page. Perhaps the confusion is that the Fawcett series was continued by Charlton running 15-21. (It then became Danger and Adventure). Charlton started a second This Magazine ... series later running #12 - 16. That was the continuation of Zaza (which itself was the continuation of Charlie Chan which Charlton took over from Comic Media). This second series became Outer Space 17-25

Such is the way with Charlton  -- an indexer's nightmare.

I may be looking at the wrong thing, as I'm not well versed in this, but I tried redownloading it and double checked the information at the bottom of page 2.  I've attached an extract from that page:

It looks to me to be from that later Charlton version you mention, I recall noticing the Comic Code emblem right off the bat.  The specific one I mean can be found here: http://goldenagecomics.co.uk/index.php?dlid=18310
