I'm wondering if anyone knows why in these books Dr. Kildare is drawn to resemble Richard Chamberlain while Dr. Gillespie is not drawn to look like Raymond Massey.

Could it be that Chamberlain gave his permission to use his likeness in the book, and was paid for that privilege of potentially profiting from use of his property (his likeness - due to the fact that he studied acting for many years, and worked at acting to build the value of his name and likenesses a popular known commodity)?  Apparently, Raymond Massey (A Canadian actor of the highest repute) felt that the pittance of money he would get from having his likeness used in a comic book was not worth it to him.  I know that there were many famous sports players that had to be paid to have their photographs, or a painted likeness placed on collectors cards for all of the major team sports in Canada and USA, and Footballers in Europe need to be paid for their likenesses to appear on collectors trading cards, and in magazines.  So, I assume that the laws regarding personal identity property, built up in a career would be fairly similar for use in printed matter as it would on collectors' cards, or on games, or in any way a 3rd party could benefit by use of a well-known person's name or likeness.
